An adult male who was rather
obese-heavy built talking very little came with his relative who told he is
having lots of anxiety and worries and is losing his temper very frequently and
is very depressed. On asking, the patient told that he is having very frequent
mouth ulcers, gets frequent runny nose, gets breathlessness on exertion.
The main complaint for which he
came to me was chronic scrotal eruptions for which he was told that he was
having filarial scrotal disease. On examination, his scrotum was swollen with
moist papule type eruptions which were discharging thin water like liquid. His
scrotal skin eruptions discharges a lot. His pant gets wet very frequently even
though he warps his scrotum with other clothes. Sometimes these eruptions do itch a bit and sometimes his scrotum
gets red, more swollen with high fever with chills and rigors, mostly at night.
He was also having hypertension. He tends to
perspire profusely and urinates profusely too. He was also intolerant to extremes of weather. Can not tolerate too high and too low temperatures.
I made the probable diagnoses of
filirarial disease along with hypertension and hypothyroidism. This is a recent
ongoing case and there is no much progress still. This case did not follow me for long time and after few visit did not return to me.
