Yesterday I was seeing patients in my dispensary when a handsome young male seated in patient seat when his turn come. I asked what are his problems and he shown me his foot. I asked for how long he is having this. He made gesture that he is deaf and dumb. He came alone making it more difficult case. Then I also started gesture conversion which I had learned a bit from a dumb and deaf co-passenger and from another regular deaf and dump patient. I came to know that it is having for 2 months and not healing despite taking lots of medicine. I enquired that does he have any blood work he responded negatively. He also revealed that it happened after a minor fire accident but he did not feel any pain at that time. I examined his feet. There was marked sensory loss. I enquired about his thirst, appetite, and urine, which all seemed within normal limit so I ruled out possibility of diabetic foot ulcer. I examined his nares and found there was crust and loss of irritation upon irritating the nose internally. I guessed his non healing ulcer is most likely due to leprosy.

Dr Rakesh,its very wise to refer this but it's good to give some homeopathic meds also along with referral n continue with ATT of DOT center. Really your Dx is perfect.
